One of the services provided under the McFarland Wellness program is case management. Case management is a method of providing services whereby the McFarland professional assesses the needs of the client, and the client's family/caretaker when appropriate, and arranges, coordinates, monitors, evaluates, a package of multiple services to meet the specific client's complex needs. This solution driven process is reported and analyzed through a data system that is designed and developed by McFarland for a specific client population, or by using software developed by the funding source.
